This PR continues refactors started with #34:

  1. Add more known server URLs; change with the type name for known server URLs, without breaking things.
  2. Expand test suite.
  3. Add retries with github.com/hashicorp/go-retryablehttp to handle potentially flakey servers.
  4. Add some more comments.
  5. Add DoH server implementation.
  6. Other odds and ends.
