Support better handling of results/xx/index.* = issue #3

This needs alignment between ptna, ptna-networks and ptna-www.

Release in sync with ptna v.2.0.0

This solves issue #70 of PTNA

Solution requires an incompatible change of CSV data entries for multiple occurrences of same 'ref', 'type' and 'operator'.

All existing CSV data has been examined and fixed accordingly.
