Collaboration model

I suggest we adopt the Integration-Manager Workflow — that is, @cbun will be the integration manager who also pushes directly to the organization repo if he chooses to, and other contributors work from their forked repos. Those of us who have admin permissions (@levinas and @sebhtml mostly) can step in to fix problems in urgent situations.

We will use the ticket system extensively for brainstorming new features and managing priorities. I find the the Writing on GitHub article useful.
