Document the procedure in #178

The first step will be just documentation and the target if 5.0 GA. After that we will implement the proper migration and tolling around it.

The targe of this card is to add documentation on what user should do on 4.3 before migrate to 5.0. Next step will be automate what we have on the other cards.
