paste_to_insert_threshold + context sensitive dictation issue #1383

I've been having problems in certain apps (like teams) on Windows 11 where the leading space added by context sensitive dictation is omitted.

After investigating, it appears to be related to the use of user.paste instead of insert at a certain length.

Creating an issue so I don't forget about it...
