Dashboard public domains create/edit does not have a field for public domain

Describe the bug
When I try to create/edit a public domain there is no input field for the name (e.g. actual public domain) to enter/edit.

To Reproduce

1. Log into the dashboard
2. Click on public domains
3. Either edit an existing public domain or click on create

Edit screen:

Expected behavior

Expected to have an input field for the public domain.

When submiting wihout entering the public domain I get an error:
