After a recent upgrade, the entityId reported by the Infrastructure agent changed. This means that the morph application in APM is no longer linked to the right host in the newrelic infrastructure monitoring.

The suggested fix for this is to set a display_name in both the Infrastructure and APM agent configurations and restart both.
