Call moderator popup

The call moderator popup:

a. Has a radio button next to the headset "What are you reporting?" - that's a heading not an option to select.

b. When selecting "Stopped playing", the Submit button (or whatever it's called) is greyed out and you can't submit it. The other options seem to work fine.
