Splitting the Bednet Distribution Activity into Individual Tasks

Presently the bed net activity on reveal web is bundled into Surveying / Dipping / giving out mosquito nets. CHAI would like to split these up into individual activities.

Considerations:

1. Short-term - update bed net distribution form with option to run a survey, unit count of ITN, and distribution.
2. Need to define the questions on each of the forms.
3. Would require Ona to enable form configuration
4. Longer-term - creating a plan template with different activities will require Ona
