ANSI writer: internal fragment links probably useless

The ANSI writer creates OSC 8 links for internal links/fragments, which is never going to be useful without extensive coordination between pandoc, pagers, and terminal emulators. That's probably out of scope for pandoc. Pandoc shouldn't create clickable links for these cases so that people don't uselessly click them.
