Rework to use Bootstrap Forms.

User story:
As a user, I want to be able to use this form on a mobile device without having to do extra pinching and scrolling so that I have a faster, more pleasant experience and maintain trust in this tool.

Current behavior:
On a mobile device, some fields and help text appear to bleed off the screen.

Desired behavior:
All fields display on mobile devices cleanly and without looking like they're going off-screen.

...except that that's a Rails gem and this is a Sinatra codebase. Hold off 'til deciding whether to proceed with current codebase or start fresh.

You should be able to just vendor the Bootstrap resources if you want. Plain old boring public/ paths are lovely.
