Currently listed as valid for use in San Francisco County. Doublecheck that, but in the meantime: http://libcloud.s3.amazonaws.com/211/08/f/610/General_Petition_Form.pdf

  • Draft field labels, placeholders and help text
  • Add fields to front end (template)
  • Add fields to back end, mapped to PDF
  • Verify and update field labels, placeholders and help text

Learned today that San Francisco County uses their own form, which gets submitted only to the Public Defender's Office (attn: Prop 47). Linked from the public defender's website but not from the superior court's, incidentally.
