As of this writing, the latest version of the form is CRM-051. Development of this project began with reference to CRM-050, so the codebase needs to be updated.

  • Compare the two forms
  • Identify their differences
  • Draft field labels, placeholders and help text
  • Add fields to front end (template)
  • Add fields to back end, mapped to PDF
  • Verify and update field labels, placeholders and help text
