Buttons and Cursor could respond to hotkey and modifier keys. This feedback would be pretty nice to have.

Changing the buttons is hard to implement, so probably not worth the effort. I also don't particularly like the flicker my periphery in GIMP, it doesn't feel good.

Changing the cursor would make sense though, it's not that hard to implement and would be nice for visual feedback.
