LMEL redirects to Dismoi form on extension uninstall

On uninstalling the Dismoi add-on (proding 3.93.5) from Firefox, the browser is redirected to https://form.jotformeu.com/92173584558367?extensionId=17d0e4bcdd61fbd0. This is the Dismoi "uninstalled" page.

We need to create a clone version of this page, branded LMEL, towards which a user will be redirected to when uninstalling the LMEL extension.

• create the LMEL version of this page
• update copy + logo
• set up this redirect on uninstall

And later change the questions to adapt them LMEL and the launch context (new issue)
