When I check a box to subscribe to a list, it adds my full email, and I would like to be able to choose which gets added.

The workflow I imagine is something like:

Set the choice somewhere
When I check a box, it adds that chosen email to the list.
When I uncheck a box, it removes all possible choices from that list.
