Automated tests only run against annotation-based identities

We are looking to deprecate the old format of Conjur authn-k8s identities, which had to be defined in a specific policy branch. To help with this, the demo should focus on provisioning applications only with annotation-based identities.

AC:

• Remove identities that used hardcoded paths to determine auth type in favor of annotation-based identities
• Remove configuration parameter that allowed for running with annotation-based identities, since this will now be the default and only identity mechanism
• Update any relevant documentation in the README about how this demo works
• (optional) Consider making it optional what type of identity the demos will use, so that this demo could be configured to run using any of the supported Kubernetes resources as the source of the app identity
