Freelook keeps getting reenabled, and I've had this issue a few times now, so I'm assuming it's either a bug or maybe hotkey I'm pressing without realizing it. Every time it's reenabled, I've only noticed it right when loading a new WAD. it then applies to all the WADs.
Also, a suggestion: I find it really confusing to have the option to disable freelook listed as "Enable mouselook". To me, "mouselook" simply means using the mouse to look, not being able to look up or down (hence "free" look), which was not possible in the original versions of Doom. Combine this with the fact that "vertical sensitivity" in the mouse settings only applies to player movement and not camera movement, and it becomes all the more thoroughly frustrating.

There is a hotkey, so check if you have it set. As far as changing the menus or names of fields: no, this port is in maintenance mode and that's what this stuff has been named for years. It's not the time to start making interface changes.
