Withdraw application doesn't work

After providing email and clicking "Send email" on step 1 of 2 of application withdrawal got redirected to 500 Server error page.

Steps to reproduce the behavior

Go to 'Candidate Page'
Click on 'Withdraw application'
Enter 'Email address' and reason to withdraw
Click 'Send email'
500 Server error after several seconds of waiting

Expected behavior

Step 2 of Application Withdrawal is shown, probably
