Authn request - Allow specifying the Issuer Format attribute

In Authn requests, some IDP require setting the Issuer Format attribute to specific values (the most common is "urn:oasis:names:tc:SAML:2.0:nameid-format:entity").

, the Issuer tag is generated without a way to specify this extra attribute. It would be nice to add a configuration item to set it.
