Search for user when inputing user before intake

name: Drew Winship
ssn: 530-61-9735
(it’s obviously fake)
when that info is typed into a form it should pop up a modal that says...
“Customer Drew Winship completed an intake at Covenant House on 1/5/2017. Do you wish to load this intake?"
the modal should have to buttons “Yes” and “No"

if no is clicked return to the intake
if yes is clicked the old intake should load
feel free to just make up an old intake for the fake "Drew Winship” customer
