Reported Issue:
Text in the form needs amending, currently reads:
"If you have a legible copy of this page (e.g. one you photographed when the indexes were open for public consultation), please use [this form] to provide your contact details so that we can invite you to send us the image."
Suggest removing the square brackets and amending the text to explain about saving the image and then attaching a screen shot.
